Community Housing Guide

This service supports an individual’s move to independent housing by assisting with transition and tenancy sustaining activities. We collaborate with the support coordinator, regional housing coordinator, housing providers and others to help the individual achieve and sustain integrated, independent living.

The Community Housing Guide can provide support with:

• Tenant Screening and Planning: Assessing individual needs and preferences, and developing a personalized housing plan.
• Housing Search and Application: Assisting with searching for affordable housing, completing rental applications, and securing necessary resources.
• Move-In Support: Providing support during the move-in process, including arranging transportation and securing essential items.
• Tenant Education and Training: Offering guidance on tenant rights, responsibilities, and household management.

CHGs help individuals find and move into suitable housing. Your CHG may not provide transportation for apartment viewings - always communicate with your CHG to see if they can assist with transportation.
